15 Minutes Scheduled

$125
This is the charge for a routine problem visit—nothing complicated. That doesn’t mean your concerns aren’t important! It just means the issue can usually be treated with simple medicines or doesn’t need a big change in care.
Examples: constipation, dry skin, acne
Chronic conditions: asthma, depression, ADHD (if medications stay the same)
If your child’s visit takes more than 20 minutes (including your provider’s work outside the room), it may be billed as a Complicated Problem Visit instead.
The 5-digit codes you see are insurance billing codes. You can use them to check your coverage.

* What’s That Mean?
Costs: This is the amount that we will bill. If your child is enrolled in an insurance policy, we will bill your insurance directly. If you do not have insurance, we offer a prompt pay discount.
Surveys: Pediatricians use surveys to check on development, like the ASQ (Ages and Stages Questionnaire), the MCHAT (Modified Autism Checklist), PHQ-9 (Psychological Health Questionnaire).
Vaccines: Vaccines are a requirement for patients of Bloom Pediatrics. You can see our vaccine policy to see which are required and which are optional. Parents can discuss the schedule with Dr Beavers, if they do not want to follow the suggested schedule.
